Practice is a
comprehensive medical
group, caring for
patients of all ages.
We focus on preventive
medicine and health
maintenance by
providing wellness
exams, routine and
sports physicals, and
patient education. We
help our patients
share in the
responsibility of
exercising good health
practices for
themselves and their
families. We provide
medical care for most
conditions including,
but not limited to,
heart disease,
diabetes,
hypertension, and
thyroid disease. We
also perform biopsy
and cryotherapy for
skin lesions, repair
lacerations and treat
many orthopedic
conditions.

patient and are
returning a call from
the office, please
remember that we
receive many calls
daily, and we
appreciate your
patience. Any
information that you
can give regarding the
nature of your call,
i.e. lab results,
x-ray results, status
of a referral, is
extremely helpful.
After Hours Calls
A physician is
available at all times
for urgent calls. If
you have an emergency,
please Call 911 or go
to the nearest
Emergency Room. If you
have a problem that is
urgent and cannot wait
until regular office
hours, please call our
answering service at
713-954-8540, and the
doctor on-call will be
paged. When paging the
doctor on-call, have
your pharmacy number
available and be sure
to turn off all “call
blocking” from your
phone. Routine calls
and prescription
refills should wait
until regular office
hours.
Insurance
At present, we
participate in over
100 insurance plans.
To expedite your visit
with us, please
designate Dr.
Vanderzyl,
Dr. Mata or Dr. Pozzi
as your primary care
provider before
arriving at the
office. We do not
accept Medicaid or
Workmen’s
Compensation.
Payment
We verify the
insurance status of
every patient before
they see the doctor.
All office fees are
payable at the time of
service. We will file
your insurance claim
for you, but patients
are responsible for
paying all
co-payments,
deductibles, and for
all non-covered
services and fees.
Forms
Sugar Lakes Family
Practice receives many
requests for forms
regarding health
status. There will be
a $25 fee to fill out
forms, including FMLA,
Disability, and
Attending Physician
Statements, unless
these requests for
forms are accompanied
by a dedicated office
visit.
